In Act 4, Scene 7 of Hamlet, Queen Gertrude delivers a poetic and sorrowful account of Ophelia's death. She explains that, in her grief-stricken madness following Polonius's murder by Hamlet, Ophelia was at a riverbank making garlands of flowers.

While climbing a willow tree, a branch broke, causing her to fall into the water. At first, her clothes kept her afloat, and she sang fragments of old songs, seemingly unaware of the danger. Eventually, however, her waterlogged garments pulled her under, leading to her death.
